We believe that having complex health care needs should no longer be a barrier to living a fulfilling and happy life. Ambition Community Healthcare aim to provide bespoke packages of care, tailored to each client’s choices. Our support workers receive specialist training in a wide variety of clinical interventions that would have once traditionally fallen under the remit of nurses. We are able to provide this level of support through our comprehensive specialist training courses and nurse-led competency process. Referrals to Ambition community healthcare can come in several ways. We have Continuing Health Care funded clients, private clients and NHS fast-track clients. We are also able to support those on Personal Health Budgets (link to PHB section). We are experienced in supporting a variety of clinical conditions, often requiring multiple complex care interventions. Our list is not exhaustive, but some examples are:
